Required for Kindergarten
According to Massachusetts State Law, it is required that your child receives a vision screening and complete physical exam within one year of enrolling in Kindergarten and be up to date on his/her immunizations prior to the first day of school.Immunizations required to enter Kindergarten:

- 5 doses DTP
- 4 does Polio
- 3 doses Hep B
- 2 doses MMR
- 2 doses Varicella (chicken pox) or physician verified date of disease
- Lead level screening (at any age)
- Most recent physical (within 1 year)
- Vision screening by doctor
There are two situations in which children who are not appropriately immunized may be admitted to school:1) a medical exemption is allowed if a physician submits documentation attesting that an immunization is medically contraindicated2) a religious exemption is allowed if a parent or guardian submits a written statement that immunizations conflict with their sincere religious beliefs.
The law states that medical exemptions must be presented at the beginning of each school year. MDPH recommends also requesting religious exemptions on an annual basis, in writing, at the beginning of each school year.
