Taft Early Learning Center Preschool, Uxbridge Public Schools Early Intervention (EI) Transition Process
A child receiving services through an Early Intervention agency may transition into the Uxbridge Public Schools on his/her third birthday, provided he/she qualifies for special education as determined through the following process:
1. The early intervention agency facilitates a release of records and sends a referral to the
Uxbridge Public Schools (UPS).
2. Following receiving the packet at UPS, the family will be directed to register their child
into our system (See the Preschool Registration information).
3. Once the child is registered with UPS and all required documentation is received, The UPS Preschool Coordinator
will attend a transition meeting with early intervention therapists and family to discuss the child's needs.
4. Once the agreed upon assessments take place, the family, Early Intervention, and school staff will meet to discuss all testing results and determine
eligiblity for Special Education based on the child's individual needs. *Please note: A diagnosis does not indicate automatic eligibility for special education.*
