TRANSCRIPTS REQUESTS: Uxbridge High School Alumni/Former students who need to request an official copy
(sealed) of their high school transcript, please complete the transcript request form
using this link.
Emailed transcripts are not official. Please download and complete the form and mail or drop off to Uxbridge High School, Guidance Office, 300 Quaker Highway, Uxbridge, MA 01569.
Appointments must be made in order to pick up your transcript. Please call the Guidance Office to schedule an appointment with Mrs. Robertson at 508-278-8633 ext. 7112. Please allow 5 (five) business days for processing. There is no charge for transcripts.
When the Guidance Office is closed for the summer or school vacations, transcripts requests will be processed when the Guidance Secretary is in the office. Please see Guidance Office Hours for the days Mrs. Robertson will be available during the summer.
